Is Vaginal Discharge with Odor Normal?
Vaginal discharge is a natural secretion produced by the body to maintain vaginal moisture and help protect against infection.
Normal discharge is usually clear or slightly whitish and may have a mild natural scent.
However, if you notice unusual odor, such as foul-smelling discharge, fishy-smelling discharge, or salty-smelling discharge, it may indicate an infection or an imbalance within the vaginal environment that should be evaluated by a healthcare professional.
What Causes Vaginal Discharge with Odor?
1. Bacterial Vaginosis (BV)
This is the most common cause of foul-smelling vaginal discharge. It occurs when beneficial bacteria, such as Lactobacillus, decrease, allowing other bacteria to overgrow.
- Grayish-white or cloudy discharge
- Fishy vaginal odor
- Odor becomes stronger after sexual intercourse
- May have little or no itching
2. Trichomoniasis
A sexually transmitted infection that commonly causes unpleasant vaginal odor and irritation.
- Yellow or green discharge
- Strong unpleasant odor
- Itching, burning, or irritation
- Pain during urination or intercourse
3. Vaginal Yeast Infection (Candida)
Yeast infections are typically associated with itching and thick discharge, but some women may also notice a musty or sour odor.
- Intense vaginal or vulvar itching
- Burning sensation
- Thick white discharge resembling cottage cheese
4. Sexually Transmitted Infections (STDs/STIs)
Certain sexually transmitted infections, such as gonorrhea, chlamydia, herpes, or syphilis, may cause abnormal discharge odor along with other symptoms.
- Lower abdominal pain
- Abnormal bleeding
- Painful urination
- Genital sores, blisters, or rashes
5. Forgotten Tampon or Foreign Object in the Vagina
Leaving a foreign object inside the vagina for more than 12 hours increases the risk of infection.
Examples include tampons, menstrual sponges, or menstrual discs/cups.
Although uncommon, this can cause severe foul odor and may lead to serious infections such as Toxic Shock Syndrome (TSS).
What Causes Fishy-Smelling Vaginal Discharge?
A fishy odor, especially after intercourse, is most commonly caused by Bacterial Vaginosis (BV).
Many women mistakenly assume they have a yeast infection and self-treat with antifungal medication, which may not improve symptoms. Proper testing is important to identify the true cause.
What Causes Salty-Smelling Vaginal Discharge?
Some women describe their discharge as smelling salty, sweaty, dried fish, or even fish sauce. This can occur for several reasons.
1. The odor disappears after washing the external genital area
- Sweat accumulation, moisture, or residual urine
- Smegma buildup between the labia or under the clitoral hood
- Wearing sanitary pads for prolonged periods without changing them
2. The odor remains despite washing
- Early-stage bacterial imbalance (BV)
- Frequent unprotected intercourse
- Certain infections
If the odor persists for several days or occurs together with abnormal discharge, medical evaluation is recommended.
Is It Serious If There Is Odor Without Other Symptoms?
Abnormal odor does not always come with itching or burning.
Many women with Bacterial Vaginosis only notice a fishy odor without any other symptoms.
Therefore, the absence of itching does not rule out infection. Persistent abnormal odor should be evaluated by a healthcare professional.
Diagnosis of Foul-Smelling Vaginal Discharge
Your doctor will review your symptoms, perform a physical examination, and assess the characteristics of the discharge.
- Pelvic examination
- Vaginal culture testing
- PCR testing for sexually transmitted infections
- Ultrasound if pelvic infection is suspected
Treatment for Vaginal Discharge with Odor
Treatment depends on the underlying cause because different odors may result from different infections.
- Yeast infection: oral or vaginal antifungal medication
- Bacterial Vaginosis: Metronidazole treatment; current guidelines may also recommend partner treatment in selected cases
- Trichomoniasis: Metronidazole treatment and partner treatment are required
- Other STD/STIs: treated according to the specific infection and usually require partner treatment
- Foreign body: safe removal by a healthcare professional followed by treatment if necessary
Avoid repeatedly self-medicating without knowing the cause, as this may delay proper treatment, contribute to drug resistance, or mask sexually transmitted infections.
How to Prevent Vaginal Odor
- Avoid vaginal douching
- Avoid scented intimate products
- Change underwear when damp
- Use condoms during sexual activity
- Reduce excessive sugar intake
- Seek medical evaluation instead of repeatedly self-treating
