Pain During Intercourse (Dyspareunia):

Dyspareunia is pain in the genital or pelvic area that can occur before, during, or after vaginal penetration.

It may be superficialpain right at the start of penetration (e.g., at the vaginal opening or the vestibule).

Or it may be deeppain felt deep in the pelvis during intercourse.

Pain During Intercourse (Dyspareunia)

Dyspareuniapain during sexis common among women and may have both physical and psychological causes.

Research from the National Institutes of Health (NIH) reports that about 821% of women worldwide experience pain with intercourse at least once, most often between ages 2050.

 

 

How common is it?

Studies by the WHO and the Journal of Sexual Medicine place prevalence at 821%.

It is more frequent during reproductive years and post-partum. Official Thai statistics are limited, but data from Siriraj Hospital, Mahidol University suggest many Thai women are affected yet hesitate to seek care due to embarrassment or believing it isnt important.

 

 

 

Causes of Dyspareunia

  • Physical causes
    • Insufficient lubrication / vaginal dryness low estrogen (menopause, breastfeeding), certain medications (antidepressants, antihistamines, some contraceptives), or inadequate foreplay
    • Vaginal inflammation/infection yeast, bacterial vaginosis, gonorrhea/chlamydia, trichomonas, herpes, cervicitis
    • Pelvic floor overactivity / vaginismus
    • Gynecologic diseases endometriosis, ovarian cysts/PCOS, pelvic adhesions, uterine fibroids
    • Dermatologic/mucosal lesions of the vulva dermatitis, herpes, abscess, lichen sclerosus
  • Psychological & relationship factors
    • Psychological stress, anxiety, fear from prior painful experiences
    • Relationship limited communication with a partner, relationship conflict

 

 

 

Diagnosis: What tests might be needed?

  1. Sexual history and physical exam to identify the site and nature of pain
  2. Pelvic exam and uterusovary ultrasound as appropriate
  3. Blood and hormone tests: estradiol, progesterone, testosterone
  4. Cervical cancer screening: Pap/ThinPrep + HPV as indicated
  5. Infection screening based on history
  6. If no physical cause is found, referral for sex therapy with a psychologist/sexologist

  • Treat the physical cause: antibiotics, antifungals, hormone therapy, topical estrogen for GSM/menopause, or surgery
